Want to join Kumataka Dojos to practice or train at one of our Dojo?

Please familiarize yourself with our Dojo Etiquette and Code of Conduct pages. You will be asked to agree to abide by them during the membership application process.

To start the process, please complete this form. Upon submission of your application you will be redirected to download the current fee schedule for the selected Dojo. We’ll then email you a members contract and schedule to print, sign and return.

Please bring the signed form with you to the Dojo when you attend your first session. The form can also be signed scanned and emailed in advance of your first session, per the instructions in the covering email.

This form must be received by your Dojo Sensei before participation in your first session.

Signatures on the form require an identified witness. (name and number detail)
Junior members require a (witnessed) parents or guardians signature.

In the form, the Next and Submit buttons will only appear once all the mandatory fields have an answer/content. These fields have a red asterisk in most cases, but not all.


Family membership: Please note a unique email address is required for each membership application. Our system(s) do not accept multiple members with the same email address.
